Telemedicine Nurse Practitioner (Remote)

About the role

Medical Director Services PC seeks an experienced Nurse Practitioner to deliver telemedicine care to nursing homes across multiple states. This fully remote position allows you to work from home while providing high‑quality clinical services to skilled nursing facilities.

Key responsibilities

• Assess patient health through virtual interviews, physical examinations, and review of medical histories.
• Document all patient interactions and care plans in electronic health records.
• Prescribe medications and perform therapeutic procedures as appropriate.
• Develop and implement individualized patient management plans to ensure continuity of care.
• Educate and counsel patients on treatment regimens, wellness, and emotional health.
• Maintain infection‑control standards and comply with federal, state, and local regulations.
• Participate in professional development activities and contribute to team knowledge sharing.

Required profile

• Current Nurse Practitioner license with active privileges in NY, NJ, GA, TX, PA, AL, or MO.
• Prior experience working in skilled nursing facilities (SNF) as an NP.
• Willingness to work 3‑12 hour shifts (AM or PM) on a remote basis.
